Job Duties
Reviews and interprets orders for inpatients using patient profiles to ensure that overdoses have not been prescribed, duplicate orders written and that incompatibilities or adverse effects will not result from combining ingredients. Contacts and confers with physicians if there are any questions or irregularities regarding patients’ orders.
Fills medication orders and prescriptions for inpatients, outpatients, nursing units and other departments of the hospital as requested; ensures timely delivery of all medications to their proper destination.
Reviews orders for intravenous medication admixtures ensuring proper dosage, concentration, compatibilities, appropriate rate of administration and labeling instructions. Contacts and confers with physicians if there are any questions or irregularities regarding patients’ prescriptions.
Ensures that adequate controlled drug inventories are appropriate and that dispensing procedures are maintained in all controlled-drug storage areas and running inventory is maintained and accurate
Reports and documents all known medication errors or adverse drug reactions.
Prepares extemporaneous medication compounds when required.
Performs monthly nursing station inspections as required.
Prepares and/or checks IV admixtures, Total Parenteral Nutrition (TPN) solutions, chemotherapy and miscellaneous sterile products as required, using proper aseptic technique with a laminar air flow hood or biohazard cabinetry.
Assessment, treatment and patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
Organizes and prioritizes daily operational activities to meet the needs of the department. Utilizes supportive personnel including technicians, interns, aides and clerks to assist in medication dispensing functions.
Maintains all required records, both legal and hospital; including controlled substances, manufacturing and packaging records. This includes participation in investigational studies as outlined by the Director of Pharmacy.
Provides drug information to physicians, nurses, patients and their families as required.
Inspects pharmacy inventory to ensure that outdated drugs, recalled drugs, and drugs that have lost their potency have been removed from stock and have been replaced as appropriate.
Maintains established departmental policies and procedures, quality assurance programs, safety and infection n control standards and works towards achieving departmental key result areas.

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures, and create necessary policies and procedures to fulfill these gaps.

OTHER JOB DUTIES
Acts as a preceptor to Pharmacy interns and as an instructor to Pharmacy extern students.
Enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops. Prepares and presents education programs to Hospital staff.
Participates in the development and implementation of future programs to improve patient care.
Participates in training programs for new pharmacy personnel including pharmacists, interns, aides and technicians.
Performs other related duties as required.
Education/Licensure
Graduation from an approved college of pharmacy or equivalent required.
Current NJ registration as a licensed Pharmacist required.
Experience:
Hospital pharmacy experience preferred.
