Facility:Children's Home Care of DaytonDepartment:Home Infusion PharmacySchedule:Full timeHours:40Job Details:The home infusion pharmacist is responsible for providing pharmaceutical care, primarily intravenous therapy, to home care patients. Consult and coordinate with physicians, nurses, third party payers, caregivers and patients in planning, providing, implementing, and monitoring effective drug therapy. This includes assessing drug orders for accuracy and appropriateness, developing care plans, maintaining medication profiles, monitoring and documenting patients' progress and outcomes, compounding sterile products, and selecting appropriate supplies.
Department Specific Job Details:
Experience: Sterile compounding experience is required.
Education:
+ Bachelor’s Degree in Pharmacy or PharmD Degree is required
Certification/Licensure:
+ Rph Ohio - Registered pharmacist - Ohio State Board of Pharmacy is required
