Job Description
We are seeking a dedicated and experienced pharmacist to join our team. The successful candidate will work onsite at our Fort Worth, TX location, providing exceptional patient care and ensuring compliance with all relevant regulations. This is a contract-to-hire position with a schedule of Monday to Friday, 10 am to 6 pm. Candidates must have at least 2 years of experience counseling patients and verifying prescriptions, and must hold a current and unrestricted license in good standing.
ResponsibilitiesOversee pharmacy technicians.Conduct comprehensive clinical reviews of pharmacotherapy for each patient, including drug utilization, counseling, compliance, drug interaction screening, side effect profiling, and drug administration support.Document results of pharmacotherapy reviews in patient charts to inform the nursing team performing follow-up assessments.Counsel patients on new drug therapy according to clinical management models to educate them on their treatment plans.Complete REMS training and follow program SOPs accordingly.Coordinate patient care with referring clinicians, addressing social, psychological, and physical needs that require customized services, and document these services in progress notes.Serve as a liaison between patients, caregivers, home health agency nurses, hospice nurses, and the prescribing physician/clinician for orders related to services provided.Provide subcutaneous teaching support for patients and caregivers, and if necessary, coordinate adequate nursing services with the prescribing clinician.Provide on-call coverage at the direction of the Pharmacist-in-Charge to meet patients' needs outside of operational hours.Process prescriptions accurately and timely.Verify the discontinuation of service for each patient by assigning the appropriate therapy outcome and deactivating the patient's chart, prescriptions, and software profile.Implement clinical management models for products and disease states.Act as the primary intake for all new team referrals initiated by telephone.Commit to providing 'outrageous customer service' as defined by internal policies.Build and foster relationships with patients and clinicians.Essential SkillsPharmacy expertisePharmacist certificationCounseling experienceAt least 2 years of experience counseling patients and verifying prescriptionsPharmD degreeKnowledge of pharmaceutical and healthcare practicesAdditional Skills & QualificationsCompliance with state, federal, and board of pharmacy lawsAdherence to policies related to government-funded payers, specifically Medicaid and MedicareAccuracy in prescription order entry and quality assurance checksAssistance in adjudication of pharmacy claimsMonitoring of hazardous waste material managementSupervision of aseptic technique of pharmacy personnelEffective use of patient education materialsReporting of adverse events and product quality complaintsMeeting or exceeding current CE requirementsSupport for professional development goals of staffCommitment to constant learning and self-education within the pharmacy arenaAttendance at conferences and reporting topics discussed at staff meetingsWork Environment
The work environment is an onsite setting at Park Vista Blvd Suite 150, Fort Worth, TX. The position requires strict adherence to company policy, particularly in relation to government-funded payers such as Medicaid and Medicare. The role involves dispensing medication pursuant to physician orders, ensuring compliance with all relevant laws, and maintaining the accuracy and quality of prescriptions. The work environment is collaborative, with a focus on providing exceptional patient care and fostering relationships with patients and clinicians. The dress code is professional.
