Summary:
The registered Pharmacist is a professional who is responsible for interpreting and dispensing physician medication orders; supervising Pharmacy Technicians; monitoring relevant lab values; calculating kinetics and dosing appropriate antibiotics and other medications; as well as initiating and maintaining TPN orders. In addition to educating patients, nurses, and other staff, the Pharmacist would be a resource for providers, nursing staff, patients and other healthcare workers regarding medications. The Pharmacist would also provide clinical services throughout the hospital including home medication reconciliation, discharge reconciliation, and medication education.

2. Performs all responsibilities/duties required by the Pharmacy as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes, but is not limited to, the age of the patient served.
Duties and Responsibilities for the following specialized Pharmacist:
A. Antibiotic Stewardship RPh
1. 0.7 FTE will be designated to monitoring, evaluating, and intervening on the management of antimicrobial therapy
2. RPh will work directly with hospitalist, pulmonology, and ED staff and infectious disease specialist
3. RPh will work with lab to develop and maintain an antimicrobial use, sensitivity, and outcomes database including an antibiogram annually
4. RPh will use the database to guide an antimicrobial formulary process to assure availability of antimicrobial agents relative to documented sensitivity patterns
5. RPh, with support of pharmacy RPh staff, will review antimicrobial orders and culture and sensitivity reports on a regular basis.
6. RPh will maintain utilization data on antimicrobial data and sensitivity and make recommendations to the P&T committee on changes in formulary in comparison to changes in sensitivity patterns
7. RPh will facilitate the Antimicrobial Stewardship Committee (ASC) including research, presentation and documentation
8. RPh will ensure the ASC reviews all TJC mandated material as required.
9. Represents pharmacy on various committees as needed and/or assigned
10. Oversees and coordinates policies and procedures relating to antimicrobial stewardship
11. Collects and analyzes data/information on quality improvement projects when indicated
12. Pursues education opportunities and /or certification programs as needed to maintain a progressive and “up to date” level of service
13. Helps with staffing duties when workload or staffing levels require.
14. Provides guidance to staff when necessary
Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

The requirements listed below must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.
EDUCATION:
Graduate of an accredited school of pharmacy required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Current Registered Pharmacist license required.
Basic Life Support (BLS) obtained within one full month from date of hire required.
Advanced Cardiac Life Support (ACLS) obtained within one year from date of hire (only for inpatient staff) required.
EXPERIENCE:
Previous clinical experience preferred.
Previous healthcare experience preferred.
2-5 years of previous experience in field preferred.
SKILLS:
Language Skills – Ability to read, write, speak, and understand the English language required.
