Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. Provide accurate and timely medication information to healthcare staff and patients. Effectively and proactively interact with medical staff regarding drug therapy issues. Document all clinical activities and interventions accurately and completely. Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges, and other approved formulary protocols and/or guardrails. Supervise and assist pharmacy technicians and other support staff effectively. Train and participate with Disaster Team drills and real-event responses as assigned. Prioritize work and lead others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency. Actively participate in staff meetings and attend staff development in pharmaceutical care to identify learning and performance needs. Support in the training and mentorship of new staff members as assigned. Serve as preceptor for pharmacy students and/or residents. Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication-related initiatives. Maintain professional competency sufficient to meet current and future needs of the facility. Maintain appropriate controlled substance records. Comply with all regulatory standards. Make appropriate evidence-based, patient-centered medication recommendations. Participate in the management of medical emergencies as appropriate, including Code Blue. Provide clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed. Essential Skills + Pharmacy degree (Pharm.D.) from an ACPE-accredited School of Pharmacy. + Active Pharmacist license in good standing with the Oklahoma State Board of Pharmacy. + BLS certification (required within 90 days of hire). + ACLS certification (required for attending Code Blue). Additional Skills & Qualifications + Doctor of Pharmacy from an ACPE-accredited college of pharmacy. + 1 year of inpatient hospital pharmacy experience. + 1 year of experience working in an ER as a Pharmacist. Work Environment Day shift - ER Pharmacist, 6A-2:30P, rotating weekends. The hospital is a full-service 180-bed facility providing a wide range of inpatient and outpatient services using state-of-the-art technology in a friendly community setting.
