Key accountabilities

Review medication orders for completeness and appropriateness Process drug orders and dispense medications to patients Counsel patients (inpatients / outpatients) on their medications and provide specialised counselling if needed Perform medication reconciliation Provide drug information to doctors, nurses and patients Assist in the preparation of ad-hoc items e.g. extemporaneous preparations, reconstituted antibiotic suspensions Other duties as assigned

Qualifications & Experience

Bachelor of Science in Pharmacy from an accredited college of pharmacy or university Registered with Singapore Pharmacy Council
