Summary:
Under the general supervision of the Director Pharmacy Services, the Ambulatory Clinical Pharmacist provides comprehensive pharmaceutical care for patients in the Mercy Medical Group Practices through integrated patient assessment and monitoring, disease-state management, population health management, teaching and pharmacy representation among the medical and nursing staff.
Responsibilities:
1.
Provides comprehensive pharmaceutical care to patients within Medical Practices.
·Works under collaborative agreement with primary care team and specialty providers
·Collaborates with other patient care team members regarding clinical interventions according to established protocols.
·Orders labs related to medication use per established protocols.
·Provides Wellness Visits in collaboration with providers
·Provides other medication related patient visits
2.
Acts as Anticoagulation Clinical Specialist for Anticoagulation (AC) Clinic.
·Provides pharmacist consulting services using established protocols under direction of the Medical Director of the AC Clinic
·Maintains and updates AC clinic protocol
·Provides education to RNs in clinic and any providers making referrals to clinic.
·Maintains quality data specific to the AC clinic
2.
Monitors medication use.
·Reviews medication lists to ensure safety, clinical appropriateness, allergy screening and interactions
·Discussed medication order clarification with prescribers.
·Maintains accurate patient records.
·As deemed necessary, works under collaborative agreement with primary care team and specialty providers
·Performs drug usage evaluations as part of CQI.
·Communicates with prescribers as necessary.
·Monitors and reports adverse drug reactions to appropriate individuals/agencies.
·Is cognizant of the cost of drug therapy and intervenes to provide pharmacoeconomic input when appropriate.
4.
Supervises/trains technicians and pharmacy students.
·Acts as a resource in day-to-day operations.
·Trains new technicians/students.
·Trains all technicians/students in new procedures.
·Checks all technician/student work for accuracy.
·Insures that technicians/students adhere to departmental policies and procedures.
5.
Maintains required records.
·Maintains accurate patient records in EHR
·Insures that records meet DEA requirements.
·Performs routine inventories.
·Audits Nursing and Pharmacy records for accuracy and completeness; intervenes as necessary.
6.
Assists in inventory control
·Provides input to Pharmacy Buyer when necessary as it relates to pharmacy products in the outpatient setting.
7.
Maintains departmental policies and procedures.
·Provides input to Director Pharmacy Services to keep policies and procedures in line with practice.
·Helps to develop new policies and procedures as necessary.
8.
Provides drug and medical information to physicians, nurses, and other healthcare professionals when requested or as appropriate.
·Provide staff development support to members of the healthcare team.
·Utilizes resources such as medical journals, texts, and the Internet to provide evidenced-based information.
·Serves as pharmacist representative on committees
9.
Performs additional duties as required or assigned.

+ Required Registered Pharmacist
