**Job Summary**
The Ambulatory Care Pharmacist works collaboratively as a member of the care management team. The pharmacist takes an active role in responding to prescription issues and advising providers on the selection, dosage, interaction and side effects of medications. Counsels patients on the medication prescription including but not limited to: proper administration, possible side effects, expected response, storage and any other questions posed by the patient. Monitors and creates care plans regarding the health and progress of patients in response to drug therapy.
In an ambulatory setting, the Pharmacist reviews and interprets provider orders, provides drug information, assists in problem solving, and provides other aspects of pharmaceutical care. Duties may consist of the following:
Navigate the electronic medical record to evaluate patient care needs.
Utilize pharmacy consults and collaborative practice agreements to assist patients in meeting their disease state goals.
Identify, resolve, and prevent potential and actual medication-related problems including untreated conditions, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
Serve as a drug information resource for providers as well as nursing and other support staff.
Manage personal and professional continuous glucose monitoring (CGM) program at assigned clinic(s) to help patients achieve A1C goals and improve overall glycemic control and patient outcomes.
Review and respond to medication questions/concerns from patients at the clinic, via telephone, or electronically through My Sanford Chart.
Conduct patient medication reconciliation including Medicare Annual Wellness Visits upon referral from providers or clinical support staff.
Assist patients who cannot afford their medications by utilizing copay cards, patient assistance programs, formulary alternatives, etc. in both the clinic and pharmacy.
Participate in Sanford Health Pharmaceutical Service relevant meetings to ensure practice and strategy alignment with various service lines.
Collaborate and closely communicate with Sanford Providers/support staff to improve patient and provider satisfaction through efficiencies and improving processes.
Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability.
Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
Understand and adhere to billing regulations and compliance.
**Qualifications**
In a retail/community pharmacy setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations.
In a medical center or institutional setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.
License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
