Pharmacist; Heme/Oncology/BMT

**Overview**
Inpatient Heme/Onc/BMT Pharmacist. 0.8-1.0 FTE.
The Pharmacist is responsible for providing medication management, clinical support and consultative services in a complex pharmacy operation. This position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement and regulatory compliance. This position may also participate in education of pharmacy students and residents.
**Responsibilities Job Description**
+ Provide clinical support and interdisciplinary education to healthcare teams, make appropriate evidence-based, patient-centered medication recommendations
+ Evaluate NCCN guidelines and clinical trials for treatment plan verification
+ Participate in all activities related to medication therapy management in BMT/hematology/oncology patients with ability to interpret levels for narrow therapeutic index drugs (e.g busulfan, sirolimus/tacrolimus, etc), able to follow protocols and make recommendations regarding conditioning regimen, immunosuppression, and supportive care
+ Staff BMT clinic and provide direct patient counseling on cellular therapies/allogeneic transplant/autologous transplant and answer all questions that patients may have
+ Identify, design, and implement improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs
+ Complete clinical reviews for patients on our service by completing medication consults related to antibiotic dosing, anticoagulation, and nutrition needs, identify drug-related problems related but not limited to improper drug or dose selection, sub therapeutic doses, over dosage, adverse drug reactions, medication use without an indication
+ Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc)
+ Support the pharmacy department’s clinical research initiatives and serve as a preceptor and mentor for PGY1 pharmacy residents, PGY2 pharmacy residents, and pharmacy students from local Colleges of Pharmacy
**Qualifications**
+ Completion of an ASHP-accredited PGY2 oncology program or 3 years of experience in oncology practice (inpatient heme/onc/BMT preferred)
+ EPIC, Beacon, and InsightRX (or similar Bayesian modeling software) proficiency
