The Pharmacist provides safe, accurate and optimal medication regimens throughout the inpatient stay while following all Federal and State regulations. **Responsibilities** **Product Verification** + Ensures medication being dispensed is accurate and in date. + Ensures provider orders are checked for appropriate dosing, interactions, and overall appropriateness. + Ensures medications are secure and not accessed by individuals who shouldn’t have access. **Clinical Services** + Provides accurate admission medication reconciliation. + Provides dose adjustments, route changes, and antibiotic recommendations. + Participates in groups and or committees relating to medications. **Development and Education** + Educates patients and staff on medications or medication classes. + Provides continuing education within the department to enhance staff knowledge. + Participates in the pharmacy college student rotation program. + Ensures proper storage, preparation, transport, and disposal of medications within the hospital. + Delegates and provide direction to pharmacy technicians in the absence of the Director of Pharmacy. **Qualifications** **Minimum Requirements** Identify items that are minimally required to perform the essential functions of this position. **Preferred or Specialized** Not required to perform the essential functions of the position. **Education:** Bachelor of Science in Pharmacy or PharmD from accredited College of Pharmacy Completion of a hospital-based or other applicable residency program **Experience:** 1 year of prior hospital experience **License(s)/Certification(s):** Current licensure in good standing with the Iowa Board of Pharmacy Valid driver’s license when driving any vehicle for work-related reasons. **Knowledge/Skills/Abilities:** · Writes, reads, comprehends and speaks fluent English. · Microsoft Office - basic skills. · Customer/patient focused. · Critical thinking skills using independent judgment in making decisions. **Other:** Use of usual and customary equipment used to perform essential functions of the position.
