Job Details Infusion Pharmacist is responsible for assisting with infusion center, as well as within our inpatient spaces. This includes order verification and oversight of hazardous and non-hazardous drug compounding, collaboration with the medical team regarding patient regimens, active involvement with service line committees, and the provision of patient education. Pharmacists fill orders for parenteral and enteral medications, monitor patient drug therapies, and provides drug information. Staff pharmacists supervise and direct support personnel. In the absence of the pharmacy management, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy manager. Responsibilities: Demonstrates accuracy in all areas of infusion pharmacy for both oncology and non-oncology medications. Compounds and dispenses pharmaceuticals both hazardous and non-hazardous, including accurately. Issues controlled substances to patient care areas and maintains records as required by law. Monitors drug therapy regimens for contraindications, drug-drug interactions, allergies, and appropriateness of drug and dose. Communicate with medical team recommendations. Reads, extracts, and interprets orders and protocols accurately. Detects and reports suspected adverse drug events accurately in a timely manner. Sustains the formulary by minimizing non formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection. Provides clinical consultation and clarification to medical team. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Excellent working knowledge of USP 797/ 800 and 795. Provides drug information to patients and families when needed. Documents all clinical activities and interventions accurately and completely. Participates in quality improvement and medication use review activities and infusion center committee if requested. Readily participates in orientation/training of new employees as directed. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures that pharmacy services are provided in a timely manner. Completes and documents monthly inspections of all assigned medication storage areas in a timely and comprehensive manner. Inspection assignments are completed without supervisor intervention. Requirements: Education: Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, an MS in Pharmacy or a Doctor of Pharmacy. Licensure/Certification: NYS Pharmacist license required. Skills, Knowledge or Abilities: Completion of an ASHP-accredited PGY1 residency or equivalent (3 years relevant experience) PGY2 or Board Certification in Oncology; oncology experience is required. Basic and advanced computer skills; proficient in the use of Microsoft Word, Excel, and PowerPoint. Experience with EPIC, Beacon, BD Pyxis / C2 Safe preferred but not required Outstanding oral and written communication skills. Able to work collaboratively on a multidisciplinary team. Flexibility with hours and scheduling to accommodate needs of the infusion center and department. Salary Range USD $59.04 - USD $88.56 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
