PRIMARY PURPOSE Provides pharmacy services to patients, nurses, physicians, and other medical staff by interpreting and evaluating the validity and safety of orders and prescriptions. Integrates core pharmacy knowledge dispensing and order review activities.
MINIMUM SPECIFICATIONS
Education
- Must have a bachelor's degree in pharmacy or Pharm. D degree from an accredited school of pharmacy.
Experience - None.
Equivalent Education and/or Experience
- None.
Certification/Registration/Licensure
- Must be a Registered Pharmacist in the State of Texas.
Skills or Special Abilities
- Must possess good oral and written communication skills.
- Must have basic computer skills.
- Must possess knowledge of drugs, procedure, equipment, common disease states and patient care monitoring within the assigned care areas.
- Required appropriate pharmacy specific mathematical skills.
- Must possess good organizational skills.
Responsibilities
1. Demonstrates effectiveness and efficiency in routine order preparation, review & verification by authorized prescribers for therapeutic appropriateness, duplications, side effects, effectiveness, lab monitoring, etc. Facilitates and ensures appropriate formulary use. Resolves routine prescribing, formulary or therapeutic issues.
2. Demonstrates accuracy & efficiency in computer entry/verification &/or preparation of medication orders/order processing/preparation including specialty orders. Ensures accurate and timely entry of medication order & processing of patient charges/credits. Promotes the use of cost-effective therapies and implements cost containment strategies such as therapeutic exchange programs.
3. Performs QM, QA and PI on a monthly basis as part of the nursing/pharmacy liaison responsibilities. Identifies & implements necessary changes and monitors the results of these programs to ensure continued performance improvement & area compliance. Provide timely information pertaining to pharmaceutical supplies, drug usage & compatibility, state & federal regulations regarding drug controls, and Joint Commission standards to designated areas.
4. Educates and communicates effectively with patients on medication therapies to ensure that patients are informed of proper drug usage and knowledge of potential side effects.
5. Acts as a resource to physicians, nurses, and patients concerning questions/support related to drug distribution & drug information services in line with hospital & departmental procedures. Actively attends & participates in departmental staff meetings & staff development programs to maintain current knowledge & understanding of procedures and/or new programs.
6. Assists in mentoring/training/education of pharmacy students, pharmacy residents, technicians on individual teams and/or in area as well as new employees and other healthcare providers. Activities should include providing both practical and didactic experiences.
7. Documents interventions for routine resolved prescribing, formulary & therapeutic issues. Identifies & discusses adverse drug events with the care team and appropriately documents information via web-based process.
8. Effective in day-to-day supervision of all subordinates (or peers if performing in a lead rph shift/area role). Provides guidance & direction to and on-site supervision of all technicians assigned to same shift/area. Ensures quality and accuracy of all activities by technicians. Assists with the selection, mentoring, and evaluation of support personnel to ensure maximum utilization of individuals while providing optimal services. Effective in professional interactions with peers & subordinates. Contributes to overall maintenance of quality & compliant pharmacy services.
9. Monitors the latest developments, advancements, and trends in Clinical Pharmacy & applicable laws, regulations, policies, laws and guidelines that impact Pharmacy. Participates and attends local, regional, national seminars/workshops, reads professional journals, and maintains licensure. Integrates knowledge gained into current work practices and shares with peers. Promotes adherence to all laws & procedures in daily practice & mentoring of peers. Seeks guidance with management as necessary to gain understanding or suggest improvements.
10. The pharmacist will complete training on the current cash collection system as well as stay abreast of current cash handling policies and procedures to maintain compliance and support to the pharmacy technician.
