We're looking for an enthusiastic, detail-oriented Medication Reconciliation Pharmacist to join our team and make a meaningful impact across our medical centers! In this pivotal role, you'll be at the forefront of our **Transitions of Care Program** , ensuring our patients receive the best care possible. You'll collaborate with a dedicated team of professionals to manage complex medication histories, provide expert guidance, and drive the quality of care through continuous education and audits. **What You Can Expect** + **Schedule:** Sunday-Thursday 12:30 p.m. - 9:00 p.m. (Initial training will be Monday-Friday day shifts for first several weeks) + **Make a Tangible Impact** : You’ll directly contribute to patient safety and care quality by reviewing and updating medication lists for admitted patients, ensuring they’re receiving the best possible care from day one. + **Expand Your Expertise** : As a key resource for Medication Historian Pharmacy Technicians (MHTs), you’ll tackle complex medication histories, sharpen your problem-solving skills, and elevate your knowledge while supporting and educating others through seminars and hands-on training. + **Leadership Opportunity** : Take charge by leading training sessions for pharmacists and MHTs, sharing your expertise on how to gather and review medication histories, and shaping the practices that will enhance the whole team’s performance. + **Ensure Quality and Excellence** : Play a pivotal role in maintaining the highest standards through regular audits, ensuring everything runs smoothly and safely while giving you the chance to fine-tune your skills in quality control. + **Drive Success Across Locations** : Lead the charge in completing an annual survey across multiple medical centers, making your mark in shaping our system’s success and securing its reputation for excellence. + **Grow Your Career** : With a variety of exciting duties and opportunities, this role provides a dynamic environment where you can continually learn, grow, and advance in your career—while being part of a supportive and innovative team.
