**Job Description** **Job Description:** This position is responsible for developing and executing pharmacy care management services, in collaboration with other disciplines within Essentia Health, to ensure the medication needs are met in the in the most safe, efficacious and cost effective manner for our Accountable Care Organization (ACO) contract enrollees. Responsible for the strategic development of an Essentia Health ambulatory formulary. This position will share accountability with Population Care Management department for pharmacy spend goals for Essentia’s ACO contracts.Additional locations are available within the Essentia Health Service area. **Education Qualifications:** Work Experience- Minimum of 1 years or equivalent pharmacist experience. Experience with managed care, formulary management, medication therapy management, geriatrics, are preferred. Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction. **Licensure/Certification Qualifications:** Licensure as a Registered Pharmacist by the Minnesota State Board of Pharmacy. Licensure as a Registered Pharmacist by Wisconsin and North Dakota State Board of Pharmacies within 6 months of hire.
