The Pharmacist, Clinical Specialist Oncology (PCSO) is responsible for: providing quality patient care in relation to the patient’s oncologic/hematologic diagnosis, prescribed treatment, age group, and other identified needs; providing comprehensive pharmaceutical care to oncology/Hematology patients to assure safe and effective drug therapy. The PCSO coordinates the drug therapy process for the oncology/Hematology patient population and the role may include any or all of the following activities: drug selection, drug information, dosing, monitoring, outcomes management, and patient education/counseling. This position requires independent decision-making, clinical judgment, and in-depth knowledge of oncology/hematology therapeutics. Board Certified Oncology Pharmacist (BCOP) credential or 10 years of active oncology experience preferred.

Responsibilities
Interprets, reviews and evaluates inpatient and outpatient oncology/hematology or other chemotherapy related patient medication orders
Performs pharmacokinetics dosing and monitoring for all patients on drugs where therapeutics levels are reported.
Participates in multidisciplinary rounds in the inpatient and/or clinic areas with direct patient care contact for visual assessment of physical and psychological signs of drug-related effects.
PCSO utilizes all Pharmaceutical Standards for Sterile Compounding. Assures timely compounding and dispensing of patient specific medications
Education/Accredited Programs
Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) Minimum of 4-years’ experience with at least 50% in oncology within the past 7 years; PGY1 residency within the past 7 years plus two (2) years of practice experience including 50% or more of that time spent in oncology or completion of a specialty (PGY2) residency* in oncology pharmacy within the past seven years, Preferred.PharmD Preferred
Licenses/Certifications/Registrations
Current, active license to practice pharmacy in New York State; or eligible for reciprocate licensure from another state.
Pharmacist Board Certification Oncology, required within 1 year of obtaining practice experience commensurate with eligibility for Board-certified Oncology Pharmacist exam.

Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $51.71 to $73.48 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
