Clinical Pharmacist - Transition of Care Specialist
$7,500 sign-on BONUS for eligible individuals
Full-time, 8:00am - 4:30pm
Job Description:
Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy.
Through various assignments within the department, the Specialists provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients within their specialty.
Requirements:
Graduate of an accredited College of Pharmacy
Current Kentucky/Indiana Pharmacist License or eligible as applicable to site
Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable) or within 6 months of start date
Current board certification or obtained within first year of hire required
Preferred Qualifications:
Completion of PGY-1/PGY-2 ASHP-accredited Pharmacy Residency in area of practice preferred or minimum of 3 years of experience in practice area
