Compensation varies based on factors including, but not limited to, experience, skills, education, performance and salary range at the time of the offer._ **Responsibilities** Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provides guidance across pharmacies and to clinical leadership. + Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. + Recommend formulary alternatives when appropriate. + Compound and dispense medications for accurate delivery to patients. + Provide drug information and education to medical staff and patients. + Communicate with physicians and other clinicians as necessary to facilitate care. + Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. + Assist with academic functions such as serving as a preceptor for pharmacy students. + May float to provide coverage of pharmacy needs. + Provide patient counseling when required. **Requirements** Licensure / Certification / Registration: + Pharmacist obtained prior to hire date or job transfer date required. Education: + Bachelor's or advanced degree in Pharmacy required. **Additional Preferences** Additional preferences. This position will be responsible for coordination of local clinical pharmacy services and formulary management, including implementation, monitoring, and evaluation of services and initiatives. In addition, it will provide support and education for local clinical pharmacy specialists and pharmacists and act as the local representative for system-wide clinical pharmacy initiatives across Ascension IL. The position will also take over as the Residency Program Director for the PGY1 Pharmacy Residency Program. **Experience Precepting Pharmacy Residents, Minimum 1 Year Required** Residency Program Director or Residency Program Coordinator experience preferred Completed PGY-1 Pharmacy Practice Residency or similar experience Licensure/Certification/Registration - Board Certification preferred Experience - 3 or more years of experience working as an acute care clinical pharmacist preferred **Why Join Our Team** Ascension Illinois delivers compassionate, personalized care throughout Chicago and its surrounding suburbs.
