Cleveland Clinic's inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians, supporting 60 nursing units, 80 operating rooms, 4 PACUs, and 10 cath labs, accounting for over 1,440 inpatient beds. **A caregiver in this position works day/evening/night/weekend 8:00am - 4:30pm.** A caregiver who excels in this role will: + Coordinate and provide pharmaceutical care for patients. + Participate in patient care rounds. + Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients. + Provide general and patient-specific drug information. + Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy. Minimum qualifications for the ideal future caregiver include: + PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy. + If hired for a specialty area where specialty residency training is available, then specialty residency is required. + Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire. + Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring. + For individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire. + For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or certification obtained within 1 year of hire.
