Pharmacy Clinical Specialist - Opioid Stewardship

The Opioid Stewardship program is an enterprise initiative to expand pain management and opioid stewardship services throughout the Cleveland Clinic Health System. The primary role of the Opioid Stewardship Pharmacist is to identify patients at higher risk for adverse outcomes, monitor safe opioid prescribing, dispensing, administering and appropriately manage pain for each individual patient. This caregiver will work with the care team members to provide therapeutic recommendations to optimize pain therapy regimens. Additionally, this caregiver will provide opioid stewardship education to providers that support safe and effective opioid use and management. This is the ideal role for a candidate with the ability to work independently. Additionally, this candidate must possess the desire and initiative to build a new service line. Positions in this role are available at Cleveland Clinic Main Campus, Hillcrest and Akron General hospitals. **A caregiver in this role works days, 8 a.m. -- 4:30 p.m.** **Occasional weekend shifts are required.** A caregiver who excels in this role, will: + Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient. + Provide general and patient specific drug information to patients, physicians, nurses and other healthcare professionals. + Interact routinely with physicians, nurses and other healthcare professionals to provide optimal pharmaceutical care to patients. + Support clinical pharmacist and technician educational development by participating in departmental staff development programs and provide leadership in the education of staff involved in providing pharmaceutical care to patients. + Support clinical pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs. + Support and participate in pharmaceutical related research in area of specialty. + Maintain expertise in areas of practice through continual learning, journal review and other educational activities. + Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy within the specialty area. Minimum qualifications for the ideal caregiver include: + PharmD or bachelor's degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training + Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire + Current Basic Life Support (BLS) certification through American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire + Completion of a post-graduate year 2 residency (PGY-2) or fellowship OR completion of post-graduate year 1 residency (PGY-1) plus two years of experience as a clinical specialist or equivalent role, which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring, including 50% or more of time spent in the specialty area OR minimum of four years of experience as a clinical specialist or equivalent role, which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring, including 50% or more of time spent in the specialty area Preferred qualifications for the ideal caregiver include: + Board certification Our caregivers continue to create the best outcomes for our patients across each of our facilities.
