Pharmacy Manager - Medication Care Coordination

Job Summary

The Pharmacy Manager - Medication Care Coordination will support infusion and clinic administered medication workflows in collaboration with inpatient infusion, outpatient infusion, and specialty ambulatory clinics across the covered Corewell region(s) (Corewell Health West/South and Corewell Health East).  Has an in-depth knowledge of pharmacy operations and pharmacy practice, including experience with outpatient infusion operations, prior authorization, managed care, medication access, medication revenue cycle, and/or leading centralized hybrid/remote teams.

Provides support for the direction, coordination, and analysis of clinical pharmacy services, focusing on pharmacoeconomic, safety, and quality outcomes. Utilizes data and information from multiple sources to identify opportunities within the medication use system. Leads a diverse pharmacy team with the purpose of improving quality and safety outcomes and managing future drug expenditures. Coordinates educational programs and activities designed to enhance clinical drug knowledge of pharmacists, physicians, nurses, other care providers. Works collaboratively with clinical/nursing leadership, physician leadership, and pharmacy personnel to implement programs and systems in support of Corewell Health strategic initiatives.

Essential Functions

Coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned. Develops a dashboard or similar tracking mechanism to monitor, evaluate, and report on clinical initiatives.Facilitates applicable committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair as assigned.Manages the activities of professional, technical and support staff with the purpose of improving quality and safety outcomes and managing future drug expenditures. Facilitates the training and continuing education of staff members. Provides direction and support to subordinate staff to ensure effectiveness and efficiency. Establishes work standards. Selects, trains, develops, and evaluates subordinates and initiates personnel actions in accordance with departmental policies and organization philosophy. Collaborates with clinical pharmacy personnel to develop training programs for new clinical pharmacists. Partners with colleges of pharmacy to coordinate experiential training activities and integrate student pharmacists into the pharmacy practice model.Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Actively participates in the development and implementation of process and quality improvement initiatives. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests. Serves as an information resource and clinical practice role model for pharmacists.Compares benefits, risks, and costs of pharmacologic regimens and collaborates with the medical staff and other caregivers to optimize benefits and minimize costs. This includes participation in drug use evaluations and other process improvement activities.Engages in continuous study of the entire professional field, including best practices, to maintain the professional competence, knowledge, and skills necessary for the satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department specific requirements.Demonstrates knowledge of safety and infection control policies and procedures, maintaining a safe and clean environment and actively participates in safety initiatives and risk mitigation measures where appropriate and completes all position and department safety related competencies and requirements on a timely basis. Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.

Qualifications

Required

Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy5 years of relevant experience working in pharmacy practice or post-graduate year two (PGY2) pharmacy residencyLIC-Clinical Pharmacist - STATE_MI State of Michigan - 90 Days RequiredLIC-Pharmacist Controlled Substance - STATE_MI State of Michigan Excludes Priority Health - 90 Days required
