The Population Health Pharmacy Technician will be responsible for assisting the MLHC Care Coordination team and Ambulatory Pharmacy team in caring for patients by providing direct patient outreach services as assigned. Promotes the safe and effective use of medications by providing the MLHC Care Coordination and Ambulatory Pharmacy teams with accurate and timely information necessary to make informed clinical recommendations related to medications and clinical quality measures. Assists patients with financial coverage (i.e., patient assistance programs). Develop and Grow your Career! Position: Pharmacy Technician, Population Health- 40 hours/week  Shift: Day Shift- 8am-4:30pm- Monday-Friday- no weekends/holiday

Experience:
1 Year experience as a pharmacy technician, Ambulatory Pharmacy Experience preferred

Education:
High School or GED

Licensures/Certifications:
Certified Pharmacy Technician (CPhT) required within 12 months from date of hire through the Pharmacy Technician Certification Board.
