Pharmacy Technician II FT - Shift Varies (Transitions of Care)

Serves a vital role in patient care and safety by interviewing patients/caregivers to obtain medication histories for patients at the point of admission, thus requiring the technician’s presence in the emergency departments, inpatient units, and/or in pre-surgery areas.  Medication history documentation is reviewed by the pharmacist, and providers use the medication history collected to determine which medications should be stopped, continued, or changed during the inpatient stay.

Uses conversational, open-ended interviewing techniques when interacting with patients; changes tasks quickly in a fast-paced, multidisciplinary environment; and shares local resources with nursing staff.  The successful pharmacy technician is also detail-oriented and documents his/her work accurately in the patient’s electronic medical record (EMR).  Ideal candidates for this position have excellent communication, critical thinking, and prioritization skills; a strong familiarity with common brand/generic drug names and available dosage forms; and are self-driven with a desire to provide face-to-face patient care every day.  Additionally, candidates should enjoy interacting with pharmacists, nursing staff, and other professionals routinely as part of a collaborative healthcare team.
