Position: Pharmacy Technician Medicine ReconciliationLocation: Evanston HospitalFull Time/Part Time: Full TimeHours: Monday-Friday, days-8:30am-5pm, e/o week-end and rotating holidays.
A Brief Overview:
Assists all healthcare professionals and patients, under the supervision of a pharmacist in the process of medication reconciliation during the admission process, discharge process and as required/requested, when there is a change in the patient's level of care. Works only under the supervision of a registered pharmacist.
What you will do:
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.Upon receiving notification, interviews patients and/or their family/caregivers as soon as possible after presenting to hospital. Obtains and records a current list of medications.Clarifies as many uncertainties and compiles as complete a medication history as possible utilizing patient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facility records and information from previous hospital encounters.Documents when the medication history is complete so that providers may reconcile for admission.Consults with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication and allergy history.Participates in the quality improvement activities of the department. Conducts quality monitors, collects data and completes documentation as assigned.Communicates with other members of the healthcare team any unresolved discrepancies, as deemed necessary.Participates in the quality improvement activities of the department. Participates in orientation, education and training programs. Reviews literature and other materials as assigned.Completes all competency/skills assessment requirements.Participates in administering annual education and competency evaluation for staff technicians.Works daily as a member of the pharmacy team and provides courteous constructive feedback to the Director of Pharmacy or Pharmacy Supervision as deemed necessary to help improve the operations of the department.Demonstrates quality customer service in all aspects of the job. Actively seeks to improve relationships and ensure a positive outcome for customer.Participates in the orientation and training of new department staff.Performs the duties of a Registered Pharmacy Technician as necessary.
What you will need:
High School Required1 Year of pharmacy experience as a pharmacy technicianCurrent License as a Pharmacy Technician in the State of Illinois Required AndCertified Pharmacy Technician (cPhT) - Pharmacy Technician Certification Board (PTCB) within 90 days of hire Required AndPharmacy Technician Certified Board (PTCB) - Illinois Department of Financial and Professional Regulation (IDFPR) Required AndExCPT Certified Pharmacy Tehnician (CPhT) - National Healthcareer Association (NHA) Required AndCurrent Pharmacy Technician license with “Student” designation issued by the State of Illinois Required
