General Summary:

Performs phlebotomy functions and non-technical duties in areas including the hospital laboratory, outpatient and off-site services.

Essential Functions: Performs phlebotomy (venipuncture, fingerstick, heelstick, and bleeding time) according to established procedure. a. Performs venipuncture using correct technique and with minimal discomfort to the patient. b. Performs fingerstick procedures with correct technique and when unable to obtain specimen by venipuncture. c. Performs heelstick on babies with correct technique. d. Performs bleeding times according to procedure. e. Performs proper hand hygiene (i.e. soap and water, or hand sanitizer) before AND after patient contact. Identifies patients and properly labels specimen tubes. a. Identifies inpatient by checking name and hospital medical record number or date of birth on patient armband and comparing this information to the LIS labels. b. Identifies outpatient by asking the patient to state their first and last name, and their date of birth and comparing this information to the registration face sheet, wristband, and LIS labels. c. Uses handheld device toscan wristband, print labels, and scan labeled specimens in to the system at the patient's chairside/bedside. d. Orders are checked to the printed specimen labels to ensure all tests are ordered and correct (i.e. a checkmark is placed next to each test on the order requisition). e. Labels all specimen tubes following established procedue: LIS barcode label, date and time of draw, and phlebotomist initials. Determines priority of STAT draws. Ensures timely transport of all patient specimens to lab. a. Prioritizes STAT request. Draws STATs within 5-10 minutes of request. b. Draws severe trauma and/or disater patients immediately. c. Ensures that STATs are transported to lab on a priority bases immediately for serious cases; Trauma, disaster, Stroke, Chest Pain; within 5-10 minutes of draw for other STATs. Hourly for routines. d. Makes conscientious effort to ensure turnaround times that meet Chest Pain and Stroke Accreditation standards. Utilizes computers as appropriate in completing assigned functions. a. Verifies all information for accuracy and checks for ABN Forms. b. Performs order entry with no errors. c. Generates collection lists and can add on or cancel tests. d. Utilizes computers and internet access appropriately. Organizes assigned work and accurately completes work load statistics. a. Draw on assigned patients by the end of shift. Helps co-workers when needed and communicates with laboratory departments when needed to ensure an orderly work flow. a. Assists co-workers with blood-drawing procedures on morning rounds before returning to phlebotomy station. b. Informs floors of missed patients or patients not drawn for any reason at time of attempt. c. Informs Supervisor of problems in obtaining specimens for any reason. d. Assists in Accessioning with pneumatic tube system, answering phones, processing send-outs, storage tracking, entering Anthem/e-Lab Corp orders, and entering lab miscellaneous orders. Under supervision will help in the training of new employees and student interns at the hospital in the performance of phlebotomy duties. a. Works with new employees to assist in training of departmental procedures, and properly completes training checklists to ensure new employee competence. b. Acts as a resource to new employees. Follows all organizational and department or unit policies and procedures. Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job related hazards. a. Attends all required safey education programs. b. Completes Education Day, required in-services, required readings, and other competency requirements in a timely manner. c. Provides complete and accurate responses to safety questions. d. maintains work area and equipment in the condition required by department standards. e. Operates assigned equipment and performs all procedures in a safe manner as instructed f. Demonstrates proper body mechanics in all functions. Responsibly follows the hospital exposure control plans/bloodborne and airborne pathogens. a. Demonstrates knowledge of techniques, procedures, and correct use of protective barrier equipment. b. Ensures a safe environment by instituting appropriate control measures. c. Attends annual education programs. d. Wears clean gloves when drawing all patients. e. Performs proper hand hygiene following glove removal and prior to leaving the department. f. Disposes of needles in special container in phlebotomy basket or in needle box in patient room. g. Follows procedures for safety dress. h. Cell phones and electronic devices are stored in employee's locker and not out in working/testing areas. Readily accepts changes in assignment. Performs work level of equal skill and responsibility as needed. May also perform work of a higher level in preparation for increased responsibility, and may be requireed to do work of lesser responsibilty when the workload of the department so requires. Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible, and courteous enviornment. a. Interacts with all of the above in a considerate, helpfule, and courteous manner, as observed by the supervisor and peers. b. Presents neat appearance in proper attire and identification, as required by the position, department, and policy. h. Continuously displays a "can do " attitude within the department and across departmental lines to contribute to the overall customer service program in place. i. Using dpeartment-specific scripting/AIDET, will answer any ringing telephone within three rings, and greets all patients/customers/employees appropriately. Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department’s/unit’s ability to meet its goals and objectives. a. Participates in staff meetings as determined by the Technical Director/Asst. Director, reads and signs minutes within 5 days for all staff meetings not attended. b. Leads and/or contributes to lean process improvement in the laboratory. c. Supports the hospital's mission, policies, and procedures through attendance and participation at committee meetings, if applicable. d. Promotes The Fisher-Titus Way philosophy: Quality, Perfection of Care, Exceeding Expectations, Delivering Value. e. Actively participates in the daily Team Huddle. f. Will lead shift change huddle, give report, review and sign pending logs with oncoming shift. g. Utilizes automated system to communicate inter- and intra departmentally, as appropriate. h. Utilizes communication boards within department to communicate issues, updates, breaks, phone extensions, etc. i. Carries portable phone at all times. j. Will access and respond to e-mail messages in a timely manner. k. Completes work assignments on time/readily accepts assignments, as observed by the supervisor. Will act as a receptionist, greet patients, and answer phones in the outpatient lab when required. a. Is courteous and friendly to patients. b. Answers phones in a prompt and courteous manner and by stating name. c. Assists with ordering testing. Is responsible for the daily cleaning of equipment and maintaining neatness throughout the department. a. Keeps phlebotomy baskets neat and check periodically for any outdated tubes. b. Will replenish phlebotomy baskets upon completion of shift. c. Will ensure that all safety and waste containers are disposed of properly. d. Maintain stock in phlebotomy room and keep phlebotomy room and storage room neat at all times, and will communicate the need for additional supplies. e. Maintains phlebotomy stock throughout the facility (i.e. Oncology, ED, Nursing Units) and records expiration date checks. Records daily temperatures of all refrigerators in the phlebotomy areas. a. Records temperatures daily. b. Notifies supervisor if temperature falls outside of established limits. Organizes assigned work and accurately completes workload. a. Organizes morning routes to include fasting labs and drug levels. b. Accepts stat add-on calls within the contracted time frames. c. Draws all assigned patients by the end of the shift. d. Calls/faxes/delivers reports to appropriate Outreach clients areas as required. e. Calls completed results to doctor's offices in a timely manner. f. Critical values are called to Physician or Clinical personnel responsible for the patient care and documented. g. Assists co-workers with additional blood-draws as necessary including add-ons and stats. h. Assists co-workers in scheduling patient draws when unavailable due to vacation or illness. i. Informs supervisor of problems in obtaining specimens for any reason. j. Updates all patients information received into hospital computer system. k. Under supervision will help in the training of new employees for outreach phlebotomy. l. Works with new employees to assist in training of departmental procedures. m. Acts as a resource to new employees. n. Maintains work area and equipment in condition required by department standards. o. Maintains outreach equipment including car/phone/supply box. Reports to work on time and meets acceptable standards for attendance. a. Adheres to the Attendance Policy by reporting to work on time and being at work as scheduled, and observed by the supervisor. b. Promotes effective working relations by following polcy regarding 15-minute break and 30-minute lunch/dinner time.
