Job Overview:

The Physical Therapist is responsible for the evaluation and treatment of patients in the L-Tach Long term care hospital and acute care settings.

Qualifications:

Required:

Doctoral Degree or Master s Degree in Physical therapy from a CAPTE accredited school or equivalent

Preferred:

1 year or more experience

Certifications and Licenses Required:

New Jersey board license and registration in Physical Therapy

Basic Life Support through the American Heart Association

Scheduling Requirements:

Essential Functions:

Arranges daily schedule as neededTakes patients at scheduled timeExhibits flexibility in dealing with daily scheduling issues Works well with front desk staff in the scheduling of patientsMakes multiple attempts to see patients when conflicts arisePerforms examinations in a timely manner and consistent with department policy (including short and long term goals)Treatment plan reflects the needs of patients identified in the examinationProvides treatment that includes but is not limited to: - Gait Training, Therapeutic Exercises, Modalities: Heat, Ultrasound, Electricity (AC/DC/Interferential/Russian), ADL Training (transfers, dressing), Assistive Device adjustment and Training, Patient/staff Education, Other treatment as defined within the scope of legal practicePrioritizes patient needs according to department policy/professional standards/state and Federal statute-regulationsShows willingness to assist other staff members in meeting their patient care responsibilitiesFor All Documentation: Notes are legible (notes and signature)All clinicians use only approved abbreviations - Physical Therapists perform and document reexaminations as necessaryEnters examinations into computer on the day it was performedCompletes progress notes on same day of serviceEnters accurate charges for each treatment performedAccurately completes the statistics form in a timely mannerMaintains all documentation necessary for reimbursement (MD scripts, visit authorizations)Supervises Rehabilitation Aides in their patient-related dutiesInsures that aides use proper techniques when assisting a therapist with a patientFollows all legal/regulatory restrictions regarding the use of AidesFollows policy related to referring patients to the Restorative Aide ProgramConsiders patient criticality, PTA knowledge and experience when assigning patients to a PTAInsures that treatment plan determined by the physical therapist is carried out by the PTAReviews and cosigns PTA notesDemonstrates understanding of the department polices related to the Student ProgramGives timely, constructive feedback to the studentReviews student documentation prior to insertion into medical recordProvides for other related experiences such as observing surgery in conjunction with the clinical coordinatorSupervises volunteers as needed and in compliance with department and hospital policiesDemonstrates compliance with findings from QI program findingsParticipates in inservices and training activities as indicatedProvides input into improving the operations of the department and the quality of care provided
