FlexStaff is a Full-Time Physical Therapist to work 1-2 months temporary assignment with potential to be hired permanently for one of our external clients, a non-profit organization that provide and manages long-term healthcare services.
Schedule: 9:00AM – 5:00PM, Monday – Friday. Weekly Hours: 35.
Requirements:
Successful completion of a Physical Therapy program from an APTA accredited university or equivalent. One (1) year of experience as a Physical Therapist and/or experience with Geriatric population, preferred.Responsibilities include, but not limited to:
Complete assessments, progress notes, treatment reviews, discharge summaries, and interdisciplinary documentation per departmental policies and procedures. Conduct participant evaluations and treatments at the PACE site and in various settings, including homes and nursing facilities. Maintain a clean and safe environment by adhering to established safety protocols. Collaborate with interdisciplinary team members to develop participant care plans and actively engage in care plan meetings in alignment with state and federal regulations. Document all relevant participant information, including assessments, progress notes, and care plans, in accordance with departmental guidelines. Record all required statistical data accurately. Consult with the physiatrist and other professionals as needed. Provide clinical instruction for Level I & Level II Physical Therapy students and Physical Therapy Assistants, as well as student volunteers. Initiate discharge planning promptly and coordinate equipment orders with team members. Contribute to in-service training programs for site and departmental staff. Engage in performance improvement initiatives and monitoring.
