Qualifications:

One of the following is required:

Current Montana Physical Therapy license

Graduate of an accredited Physical Therapy program with ability to obtain Montana licensure within 30 days of hire

Current HS AED CPR certification

Strong organizational skills, attention to detail, and ability to set priorities

Ability to work both independently and within a team environment

Excellent verbal and written communication skills to interact with various audiences

Strong interpersonal skills, demonstrating professionalism, confidentiality, and diplomacy

Proficiency in Microsoft Office Suite and willingness to learn additional software as needed

Consistent attendance and reliability as scheduled by department leadership

Responsibilities:

Develop, implement, and revise individualized, goal-directed physical therapy plans based on patient needs

Perform comprehensive assessments evaluating functional, physical, mental, emotional, and environmental status

Maintain an updated plan of care, including goals, interventions, and visit frequency

Effectively plan, communicate, supervise, and delegate to ensure efficient therapy services

Complete all patient treatment according to physical therapy standards and department policies

Conduct admission and discharge evaluations, daily assessments, and identify patient-specific goals

Ensure accurate and timely documentation in accordance with Medicare, Medicaid, and private insurance guidelines

Participate in rehabilitation programs and projects within the department and community

Meet set productivity standards as defined by the department

Schedule:

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
