We are looking for a full-time PT to add to our team! The PT will be responsible for managing their own case load and working with worker's compensation patients along with assisting with Hospital in Your Home.
The Hospital in Your Home experience is designed to provide hospital level care in a patient’s home environment. This allows for deeper engagement between the clinical team and the patients, a consistent care team through phases of care, address goals of care and underlying social determinants of health and care that is designed and centered around the needs and wants of the patient. This is an experience we strongly believe in, and one that aligns with our own mission, vision, and values. We are looking for a Physical Therapist (PT) who equally supports this experience and is enthusiastic about replicating the capabilities of a brick-and-mortar hospital with in-home services.
Physical Therapist (PT) Responsibilities:
Evaluations patient's need for ongoing physical therapyProvides physician prescribed physical therapyImproves or minimizes residual physical disabilities of the patientReturns the individual to optimum and productive level within the patient’s capabilitiesParticipates with all other Hospital in Your Home personnel in patient care planningPerforms all skilled procedures as ordered by physicianConsults with physicians regarding change in treatment via telecommunicationPrepares clinical and progress notes and turns notes in within 4 hours of completed visitPhysical Therapist (PT) Requirements:
Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy AssociationCurrently licensed in the state of KansasAbility to exercise initiative and independent judgmentReliable car and auto insuranceValid Driver's LicenseCurrent CPR Certification (BLS- Red Cross or American Heart Association) *we offer a class at a reduced cost if you need your BLS*Negative TB test within last yearClean background and criminal recordLaptop or Tablet that can be used in patient's home for charting purposesCOVID VaccinatedUnderstands regulatory compliance with state and federal regulationsAdhere to HIPAA and maintain client confidentialityThis is a full-time opportunity!
