Summary:

Plans and administers physical therapy treatment in inpatient, outpatient or community setting for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to restore function, relieve pain, and prevent or mitigate disability by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.


Critical Tasks:

Maintains exclusive focus upon patients/residents, the quality of their care, and their individual needs without significant distraction by administrative and non-patient contact activities.Provides inpatient or outpatient physical therapy services including screenings, consultations, evaluations and ongoing physical therapy treatments.Performs and interprets evaluations of patient physical limitations and activities of daily living abilities.Reviews Physician’s referral (prescription) and patient’s condition and medical records to determine physical therapy treatment required.Tests and measures patient’s strength, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise treatment programs.Plans and prepares written treatment program based on evaluation of patient data.Administers manual exercises to improve and maintain physical functionality.Instructs, motivates, and assists patient to perform various physical activities and in use of assistant and supportive devices such as crutches, canes, and prostheses.Administers treatments involving application of physical agents.Evaluations and accurately documents patient progress and continually adjusts treatment methodologies accordingly.Develops, adjusts and manages individual patient plans of care.Consults with other members of rehabilitation team to select activity program consistent with needs and capabilities of individual and to coordinate physical therapy with other therapeutic activities.Selects constructive activities suited to individual’s physical capacity, intelligence level, and interest to upgrade individual to maximum independence, assists in restoration of functions, and aid in adjustment to disability.Provides and trains patients in the use of durable medical equipment, prosthetics and orthotics.Administers massage, manual therapies, dry needling and traction to increase the patient’s strength, and decrease or prevent deformity and crippling.Uses therapy modalities such as heat, ultrasound, electrical stimulation, and paraffin as required.Conducts training programs or participates in training other workers in physical therapy techniques and objectives.Directs and assists patients in exercises and use of mobility devices and orthotics.Educates patients and families in appropriate therapy methods.Maintains high productivity and consults/serves 8-16 patients per shift.Provides timely medical documentation in accordance with all regulatory and payer requirements.Participates and consults in interdisciplinary therapies care coordination and program discharge planning.Complies fully with all regulatory and information privacy standards and mandates.Supervises and provides effective training to multiple employees.Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.Demonstrates ability to clearly communicate, both orally and in writing, while performing all essentials functions.Performs other special projects and duties assigned.

Required Qualifications:

Master of Science Degree in Physical Therapy.Arizona Physical Therapist license in good standing.Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.Employment at the Caring House requires: AZ Department of Public Safety Fingerprint Clearance Card.
