Position Purpose: Trinity Health PACE seeks a dedicated Physical Therapist to deliver clinical evaluations and treatment in acute, restorative, or maintenance settings across our Day Center, Nursing Home, or in-home environments. Your role is pivotal in helping participants achieve optimal functional independence and well-being.
What You Will Do:
Conduct comprehensive physical therapy assessments and develop individualized care plans.Provide therapeutic interventions to participants with diverse disabilities, promoting maximum functional independence.Collaborate with interdisciplinary teams and assist in managing durable medical equipment needs.Train staff in safe body mechanics and use of therapy equipment.Maintain high standards of documentation, safety, and quality assurance.Minimum Qualifications:
Bachelor's degree in Physical Therapy from an accredited program.Active, unencumbered physical therapy license in the state of North Carolina.At least one year of experience working with frail or elderly populations; less experience requires PACE-specific training.CPR and First Aid certification may be required.Demonstrated ability to work independently, manage conflict, and communicate effectively with diverse groups.Position Highlights and Benefits:
