Location: New Hartford, NY Category: Gan Kavod - OPWDD Housing Posted On: Thu Dec 5 2024 Job Description: Discipline: Physical Therapist Supervisor: Program Director/ Assistant Program Director Schedule: Monday to Friday generally 8-4 Location (s): 6 Gan Kavod IRA's Salary: $68K-72K Responsibilities: Evaluate present level of functioning and future goals for individuals residing in the Gan Kavod IRA program, communication identified physical therapy needs in a clear and concise manner to the team via reports quarterly, semi-annually, and when interim changes in condition or treatment necessitate. This may include, but is not limited to, recommendations for appropriate adaptations/equipment for use at home or day program; and/or recommendations for accessing community resources. Performing the above services for individuals participating in community day programs when presenting concern is unique to the IRA home environment. In conjunction with Program Service Specialist, serves as clinical liaison with coordinating discipline at day program when individual physical therapy needs overlap. Participates either by attendance at or through written communication prior to the Life Plan meetings to address individuals physical therapy needs. Recommends equipment and/or supplies appropriate to meet resident needs, and when inventory demonstrates the need to reorder. Participates in program development and revision within the IRA. Maintains contact at least weekly with Program Service Specialist to keep abreast of individual's needs. Communicates potential recommendations with Program Service Specialist prior to finalizing reports for submission. Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws. Performs other therapy related duties as assigned or requested by supervisor Job Requirements: Qualifications: Minimum Bachelor of Science degree in Physical Therapy from an accredited program, with at least one (1) year of experience, through practice, academic curriculum and/or clinicals, with developmentally disabled or geriatric individuals; and current New York State Physical Therapy license. Demonstrated knowledge of, and rationale for, physical therapy techniques, modalities, equipment use and exercise programs. Additional Information: Charles T.
