Job Objective: \n

The Physical Therapist Assistant will provide physical therapy services under the direction of a physical therapist in a professional manner in accordance with approved methods and standards of practice, adhering to the guidelines of the American Physical Therapy Association\n
\n

Minimum Qualifications:\n

Education: Has successfully completed a training program approved by the American Physical Therapy Association. Must meet all requirements for the applicable state regulatory agency. Ability to read, write and communicate in English.\n

Experience: Minimum of (1) years in a home health care agency\n

Licensure: Certification as required by the State of Connecticut\n
\n

Essential Job Functions:\n\nTreatment regimens as directed by the physical therapist related to dysfunction of the neuromuscular and musculoskeletal systems.\nTreatment may target any of the following areas but may not be limited to strength, range of motion, mobility, balance, gait, coordination, endurance, wheelchair use, equipment recommendations, body awareness, orthotic and prosthetic needs, body mechanics, posture, relaxation\/stress management, vestibular dysfunction, cardiovascular condition, home exercise program, wound care, and isokinetic training.\nPerforms and documents the client's level of function and participation in the development of the total client care plan.\nIdentifies the client and family needs for home health services and referral for same when needed.\nParticipation in case management conferences\nInstruction of the client, family and other agency personnel in the patient's treatment regime when indicated.\nAt least every (30) thirty days will confer with the registered physical therapist and shall document this conference in the client's clinical record. This conference shall include a review of the current care plan and any appropriate modifications to the treatment regime in accordance with changes in the client's status.\nAssures quality and safe delivery of therapy services to all clients.\nPerforms and submits timely documentation on all appointed visits.\nAssures compliance with infection control and safety instructions.\nKeep current with required CEUs and monitor staff for compliance with required CEUs.\nActively participates in the communication process integral to ensuring quality client care.\nFollows established lines of communication and authority.\n
