When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services. The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.
