Under the direction of the Rehabilitation Director, evaluates, plans, and administers medically prescribed physical therapy treatments to inpatients and outpatients.
Essential Job Functions:
Performs a comprehensive physical, psychosocial, and safety assessment of assigned patients as evidenced by clinical documentation
Develops a Plan of Care to promote optimum function and safety as documented in the clinical record
Coordinates patient care with other providers ensuring optimal patient care and effective communication as noted in the clinical record
Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.
Follows hospital exposure control plans/blood borne and airborne pathogens
Maintains confidentiality of all hospital and patient information at all times. Follows HIPAA regulations and policies
Regular and predictable attendance is an essential job function
Communicate with patients, families and co-workers in person, in writing and on telephone
Minimum Qualifications:
BS Degree in physical therapy (PT)
Current state PT license
CPR certification
Basic understanding of physical therapy services across the spectrum of inpatient and outpatient care
