Position Summary:
The Physical Therapist is responsible for assuring that the PNHS values of Head + Heart, together, are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, outcome/compensatory assessments, compliance with regulatory standards and participating in process improvements to advance patient care outcomes and satisfaction.
The Physical Therapist will communicate in a timely, clearly, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers. The staff will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Therapists will practice in compliance with professional standards of practice and professional code of ethics.
The Homecare Physical Therapist acts as a case manager to access and evaluate patient care needs and provides skilled therapy care and teaching to patients and families in their home. The Physical Therapist is responsible to improve and prepare patients and families for self-directed and/or assisted care in their community setting. The Physical Therapist coordinates medical, social, and financial resources to develop a comprehensive plan of care. The therapist supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery.
Work Schedule: M-F, occasional weekends (~every -2 months)
Required Qualifications:
Education, Experience or Equivalent Combination: Bachelors, Masters, or Doctorate degree in Physical Therapy Licensure/ Registration/ Certification: Current license with Minnesota Board of Physical Therapy Knowledge, Skills, and Abilities: Demonstrates knowledge of age-related growth and development principles to provide appropriate service regarding primary diagnosis and multiple comorbidities affecting care. The role will require proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record system, OASIS evaluation and data setPreferred Qualifications:
Work experience as a Physical Therapist for at least 3 years preferred. Experience in homecare desirable with knowledge of OASIS data set. High level assessment skill including medication reconciliation and teaching, technical skill, and communication skill.
