PHYSICAL THERAPIST - HOME HEALTH at Providence Medford Medical Center

Schedule: Full Time, Day shift 
Work Location: This position will be primarily in Jackson County with an occasional visit needed in Josephine County on a Saturday

Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence.

Required Qualifications:

Bachelor's Degree From an accredited Physical Therapy Program. Or Master's Degree   From an accredited Physical Therapy Program. Or D.P.T. From an accredited Physical Therapy Program.    Within 30 days of hire: National Provider BLS - American Heart Association Upon hire: Oregon Physical Therapist License  Upon hire: Washington Physical Therapist License - if practicing within this state 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.
