The Mary T Family of Companies is seeking at part-time Physical Therapist to work with our Home Health clients in the areas of North Branch, Pine City, Rush City, Braham, and Cambridge, Minnesota.
The Physical Therapist is responsible for evaluating needs of referred patients, formulating treatment plans in home care environment, and providing physical therapy services defined in treatment plans. The position works cooperatively with RN case managers, physicians and other home health staff and supervises the physical therapy assistant staff.
ESSENTIAL FUNCTIONS AND BASIC JOB DUTIES
Examine and evaluate individual patients and develop a treatment plan to improve mobility, reduce pain, restore function and prevent disability. Establish appropriate treatment goals of individual patients. Implement physical therapy Point of Care by administering exercises, training in use of assistive devises, instructing, encouraging, and assisting patients in performing physical activities to improve ambulation, functional activities and actives of daily living. Document patient care by charting in patient’s record. Supervise physical therapy assistants. Confer with the patient, medical practitioners, and appropriate others to plan, implement, and access the intervention program.SCHEDULE:
This position is part-time, offering 16-24 hours per week. The schedule can be flexible and set up by the employee. Visits will take place during daytime hours (8a-4p), Monday - Friday. No weekends required!

Master’s Degree in Physical TherapyCERTIFICATION/LICENSURE:
Current Physical Therapy License in Minnesota Valid driver’s licenseEXPERIENCE:
Minimum one (1) year experience in home health, a short-term rehab, outpatient, or hospital inpatient rehab setting.KNOWLEDGE, SKILLS, ABILITIES:
Strong communication, organizational, collaborative, and prioritization skills Ability to build and maintain trusting relationships with patients, employees, and outside entities. Ethics and integrity with advanced problem solving skills Demonstrated knowledge and skills necessary to provide care and communicate with primarily the geriatric population Demonstrated knowledge of the principles of growth and development over the life span. Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements to his or her age-specific needs. Basic computer and math skills Leadership skills with ability to coach and train staff Knowledge of and ability to practice safe patient handlingWHAT WE HAVE TO OFFER YOU:
