Role and Responsibilities
A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physicians orders.
Essential Functions
+ Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
+ Prepare an assessment of the patients condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
+ Recommend modification of plan of care to physician, as needed.
+ Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
+ Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
+ Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).
+ Include and educate the patient and caregiver(s) regarding treatment plan and goals.
+ Attend physical therapy staff meetings and in-service presentations.
+ Participate in case conferences.
+ Participate in the development and periodic revision of the physicians Plan of Treatment.
+ Assure appropriate safety practices and maintenance of equipment.
+ Other duties as assigned.
