Physical Therapist - Homecare & Hospice, Per Diem

Job Description:

**This position will serve patients in the Augusta area and surrounding towns**

Performs initial and ongoing assessments of patient's condition. Designs the appropriate plan of care, including type and frequency of treatment.Implements the treatment plan using appropriate modalities. Documents subjective, objective, and assessment findings and plans patient progress.Educates and counsels the patient and family/caregiver. Plans and/or assists with patient discharge from therapy services and treatment setting. Provides information regarding appropriate selection/use of adaptive equipment and support programs.Supervises and delegates the care provided by physical therapy assistants according to established licensing laws, state regulations and practice standards.

The ideal candidate will have:

Current licensure by Maine State Board of Examiners in Physical Therapy or ability to do so at time of hire is required. At least 1 years of PT experience is requiredValid state of Maine driver’s license with acceptable driving record
