Physical Therapist Boulder Community Health, Boulder, CO 80303 $41.42 - $62.15 an hour - Per Diem, Days. Various hours a week. **Range includes per diem differential. Boulder Community health is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provisions of physical therapy services. Must have experience evaluating neurologically involved patients and conditions. Developing, executing, assessing, and modifying physical therapy intervention as it applies to the neurologically involved patient population. Responsibilities •Evaluative skills: Selects appropriate tests and measures to determine goals and problems course of treatment and appropriate therapeutic procedures/modalities. •Selects and establishes physical therapy diagnosis. •Planning skills: Formulates and maintains an individualized patient plan of care based on long and short-term goals. •Implementation skills: Provides treatment using appropriate tools and demonstrates a complete knowledge of patient management. •Assessment skills: Evaluates patient response to treatment and revises plan of care as indicated. •Serves patient population from adolescent to older adult/geriatric. Is able to modify assessment, treatment and interaction skills based on the age and developmental level of the patient. •Promotes effective communication and teamwork with team members, the hospital and other contacts. •Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress. •Initiates and is responsible for problem solving and decision making with relation to patient care. •Functions as an advocate for the patient About Boulder Community Health and Boulder, Colorado Boulder Community Health’s vision is partnering to create and care for the healthiest community in the nation.
