Physical Therapist - LV Craig Family Medicine Clinic - Full Time 10 Hour Days (Exempt) (Non-Union)

Under general supervision and in accordance with standardized procedures, incumbent performs physical examinations and treats common episodic and chronic physical therapy health problems in the clinic, counsels' patients and families and monitors patient progress. The Physical Therapist develops and implements physical therapy services and directs patient care in coordination with the inter-disciplinary team and in compliance with organizational goals and appropriate regulatory agencies. Provides physical therapy care and evaluation services to pediatric, adult and geriatric patients with various diagnoses. Major duties include patient evaluation, treatment planning for effective patient outcomes problem resolution, patient care documentation, attendance at patient care related meetings, delegation of duties to support personnel, participation as a member of a team, maintenance of an appropriate level of productivity, and participation in performance improvement activities. internship.

Essential Duties:

Functioning as a primary care provider or upon referral of a patient by a physician or qualified NPP to the Department, the therapist initiates care per policy evaluating the condition, establishing a treatment plan, providing interventions, and making appropriate referrals. Intervention may include recommending or issuing assistive devices, braces, and other medical devices as allowed by Physical Therapist Scope of Practice.Conducts in-depth interviews with patients and/or family, constructs comprehensive medical histories, performs initial and periodic physical examinations and explores patient’s interpretation of health needs. Identifies and treats patients’ physical medicine needs. Manages treatment of physical therapy patients.Supervises and directs Physical Therapist Assistants and/or Aides as necessary for patient care.Directly performs or orders appropriate diagnostic examinations within scope of practice and interprets finished results.Employee manages all aspects of physical therapy evaluation and intervention.Develops and implements long and short-range treatment plans.Initiates consultations and monitors scheduling of patients for special tests.Employee consistently completes all required documentation as per departmental policy.Employee participates in clinic and rounds as patient care responsibilities allow.Attends and contributes to patient care conferences, as listed above to provide continuity of care or if unable to attend, provides written progress/plans for each patient to the referring provider (or other providers participating in care) in a timely manner.For all patient serviced by discipline provides education and makes recommendation to other healthcare professionals on the team regarding discharge plans and/or modification to the plan. Assumes responsibility as lead in assigned clinics and coordinates all aspects of communication to and from physician, NPP, and therapist.Employee effectively manages a full patient caseload, which includes all aspects (i.e. patient care, documentation and communication, and assists in meeting departmental efficiency requirements).Instructs and counsels patients and families regarding preventive care, physical medicine problems, and use of prescribed treatments and drugs. Recommends consultation with other providers as necessary.Able to be flexible with last minute scheduling changes and prioritizing. Able to be lead PT, while still maintaining caseload.Thorough organization of unscheduled non –productive time (i.e. Patient not available, outpatient cancellation, etc.), able to improve department efficiency (i.e. Assists other staff, projects, other documentation), while still maintaining a full caseload.Employee exhibits skills to provide care in a variety of PT disciplines/settings (i.e. Acute, rehab unit, or outpatient in the areas of orthopedics, sports, cardiac, wound care, oncology, and neurologic physical therapy).Employee assumes staffing of unit and patient care delivery as lead PT when needed.After one year, provides education for and evaluates performance of at least 1 student for a cumulative minimum total of 10 weeks and receives satisfactory evaluation from the SCCE.After 6 months within the organization, provides education for and evaluates performance of new staff members competencies or registry via orientation, check off.After one year of experience, serves as a clinical instructor for first year Physical Therapy interns for a 2-16 week rotation.Provides formal education to internal or external departments (i.e.-therapy department, outpatient-surgery, residents, etc.)Attends one professional meeting per year (district, state or national) and is a member of the APTA.Demonstrates commitment to the profession as evidenced by membership in the APTA and attends a professional meeting.Publishes, speaks, participates in clinical research or runs for elected office committee member in the APTA.Employee assumes responsibility for a special project as assigned by Clinic Director or chief.Performs other duties as assigned.

Required Qualifications:

Req Master's degree Physical TherapyReq Must demonstrate effective verbal and written communication skills, and mature and sound judgment.

Preferred Qualifications:

Pref Ph.D. or equivalent doctorate Physical TherapyPref Experience as a Physical Therapy preferredPref Residency or fellowship training in a physical therapy specialty is preferredPref Board Certified Board certification in a physical therapy specialty is preferred

Required Licenses/Certifications:

Req Physical Therapist Current and valid license to practice Physical Therapy or eligible under Nevada and must obtain within 90 days post hire.Req Basic Life Support (BLS) Healthcare Provider from American Heart Association.Req National Provider Identifier (NPI) Must have current and valid National Provider Identifier Number (NPI)Req Must obtain USC Credentialing from governing board within 6 months of hireReq Fire Life Safety Training (LA City) Fire and Safety Certification. If no card upon hire, one must be obtained within 30 days of hire, and maintained by renewal before expiration date. (Required within LA City only)
The annual base salary range for this position is $81,120.00 - $133,010.00.
