Summary :The Physical Therapistdiagnoses and treats patients with medical problems or other health-related conditions that limit their ability to move and perform functional activities in their daily lives. The physical therapist provides a variety of health care services centered on the unique and individual needs of each patient according to the plan of care, using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. Works with a multidisciplinary team to deliver a full range of services, and participates in team-based continuous improvement.

Job Description:

KEY ACCOUNTABILITIES:

Demonstrates best practice and accountability in the performance of safe and effective assessment, reassessment, development, and implementation of the patient’s therapy plan of care. Consults with other disciplines, as required, to coordinate patient care services.

Delegates appropriately to physical therapy assistants and rehabilitation aides to support patient needs while maintaining accountability for the care plan. Understands the competency level of those delegated to, and supports learning and teaching opportunities to further enhance the skill set of care team members.

Completes accurate and timely documentation of services provided in accordance with professional and compliance standards to ensure accurate billing and coding to maximize reimbursement and compliance.

Assists in the development of standards of care, preferred practice patterns, protocols, and clinical procedures using evidence-based practice to ensure quality care.

Demonstrates responsible actions and initiatives to positively impact patient access to care and contribute to the department’s financial performance.

Promotes successful relationships with referral sources and within the community to enhance strategic growth and gain market share.

Fosters clinician autonomy through individual professional development and mentorship to support the team’s success in advancing their profession.

Develops and/or implements clinical and community-based activities that improve or expand services within the department, and/or ThedaCare to benefit patient care.

Proactively promotes putting the patient and family first through facilitating positive patient experiences.

QUALIFICATIONS:

Bachelor’s degree or higher from an accredited school of physical therapy

American Heart Association Basic Life Support (BLS)

License to practice as a physical therapist in the State of Wisconsin

PHYSICAL DEMANDS:

Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of fifty (50) pounds without assistance

Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:

Occasional exposure to sharp objects and instruments

Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock

Occasional high noise level in work environment

Possible exposure to communicable diseases, hazardous materials, and pharmacological agents

Occasional contact with aggressive and or combative patients

Occasional exposure to heat sources such as hot water for splinting or heat gun

Schedule: Mon-Thurs from 7:00 am – 5:30 pm (Outpatient) at ThedaCare – Ripon.  Weekend rotation is a maximum of 1 in every 4 weekends (Inpatient) at ThedaCare – Berlin and Juliette Manor at Berlin
