Job Title: Part Time Home Health Physical Therapist
Location: Redmond, OR

Compensation is a Pay Per Visit Model, Non-Visit Rate at $52/hour with Starts of Care starting a $150, Open to negotiation!
Job Description: Are you a dedicated Physical Therapist looking to combine your love for people and strong work ethic with an opportunity to advance your career?
As a Physical Therapist with us, you will:Provide physical therapy assessments, services, direction, and care to patients in their homes.Evaluate the patient's home environment and functional status.Participate in the development and implementation of individualized care plans.Work collaboratively with an interdisciplinary team to ensure optimal patient outcomes.Educate patients and their families on continued care and functional activities.
Responsibilities:Conduct comprehensive physical therapy evaluations.Develop personalized treatment plans based on patient assessments and goals.Implement therapeutic interventions to improve mobility, strength, and overall function.Monitor patient progress and adjust treatments as necessary.Document patient care and progress notes accurately and timely.Communicate effectively with patients, families, and other healthcare team members.
Qualifications:Valid Physical Therapist license in the state of Oregon.Minimum of one year of experience as a Physical Therapist, preferably in a home health setting.Strong assessment and clinical skills.Excellent communication and interpersonal skills.Ability to work independently and manage time effectively.Reliable transportation and willingness to travel within the service area.
