Physical Therapists are primarily responsible for planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal and state standards, guidelines, and regulations. To ensure the highest quality of resident care can be maintained. Physical Therapists must be able to evaluate a patient’s condition and devise a customized rehabilitation and treatment plan. The Physical Therapist’s goal is to develop, maintain and restore maximum movement and functional ability, in order to maximize quality of life.

Education, Experience, and Licensure/Certifications

Must have an active state license or license eligible and meet the educational requirements as defined in the job description for that license type.

Bachelor of Science, Master of Science or Doctorate in Physical Therapy from an accredited program.

Responsibilities include but are not limited to the following. Additional duties may be assigned.

Plan, develop, organize, implement, evaluate and direct physical therapy services as well as its programs and activities in accordance with current regulations and guidelines that govern the skilled nursing facility.

Maintains and completes accurate documentation, department records and reports.

Ensures effective communication regarding PT programs.

Initiates referrals when appropriate and in accordance with Physical Therapy Department procedures.

Conducts screenings of residents at regular intervals to determine need for intervention/treatment.

Interprets and communicates evaluation findings to residents, family and other team members.
