Exceptional Rehab at Dover Care Center in Dover, TN is seeking a PRN Physical Therapist ESSENTIAL DUTIES AND RESPONSIBILITIES: Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities. Performs initial resident examination, evaluation, interpretation of referrals, and creates the initial plan of care for each resident and revises as appropriate. Provides on-site assessment and re-evaluation of each residents treatment per the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate. Coordinates discharge plan and decisions with the PTA. Ensures proper physical therapy care rendered by the PTA for all residents for whom he or she is the therapist of record by delegating appropriate portions of the treatment plan and program to the PTA consistent with the PTA’s education, training and experience; being accessible by telecommunication to the PTA at all times while the PTA is treating resident for whom he or she is the therapist of record; providing supervision of the PTA as required by State Practice Act, and as needed. provides supervisory conferences and visits as required and as requested. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department. Communicates effectively with multi-disciplinary team to develop plan of care and advocate for patient. Demonstrates effective interdisciplinary communication regarding resident treatments and discharge planning. Cooperates with all facility and corporate departments. Must be able to relate professionally, positively, and cooperatively with patients, patients’ family members, and other employees at all levels. Maintains proper documentation. This includes but is not limited to: creating and maintaining a resident record for each resident treated; completing documentation according policies including timeliness and medical necessity support, and adhering to state, federal and third party billing regulations, assuring therapy billing and labor entries are complete, accurate and timely. Participates in quality and process improvement activities, as directed. Participated in continuous survey readiness, as directed. Presents with a professional appearance and attitude at all times. Adheres to facility dress code if applicable. Maintains a clean, safe and professional department, and maintains regular attendance. Maintains and applies current skills and knowledge through continuing education and in-service programs. Participates in in-service training and completes all mandatory training within designated time frame. Maintain knowledge of Medicare Policies and Regulations, LCDs, and NCDs that directly impact care. Participates in census building and marketing activities. Participates in clinical program development and implementation. Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures. SUPERVISORY RESPONSIBILITIES: Directly supervises PTA's and carries out supervisory responsibilities in accordance with the facility policies, practices, procedures, and applicable laws. Responsibilities include: interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Indirect supervisor of nurse, program, activity and other assistants as their job functions relate to resident care. Must consistently use discretion and independent judgment. EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED) preferred. Meets state and federal requirements. Long term care and EMR experience strongly preferred. CERTIFICATES, LICENSES, REGISTRATIONS: Graduate of an accredited education course in physical therapy. Licensed in the state in which they will be practicing. Driver’s license per position requirements. Full Time - $15,000 Sign on Bonus! Part Time - $7500 Sign on Bonus
