Responsibilities Responsible for the provision and documentation of patient care, including clinical evaluation and the formulation of treatment plans & goals. Provide physical therapy treatment to patients. Formulate & carry out discharge plans. Supervise assigned staff. Qualifications Job Requirements: Baccalaureate, Master, or Doctorate Degree of Physical Therapy. Current Texas Licensure to practice Physical Therapy. CPR Certification. Preferred Job Requirements: Physical Therapy experience in an acute care hospital and/or in a rehabilitation facility with participation in a multi-disciplinary team.
