Position Purpose:

To be a medical provider and leader in the clinics of HPMG as we pursue perfect care, service and access at an affordable cost, following the STEEEP principles of safe, timely, effective, efficient, equitable, patient centered care.

 

Reporting Relationship:

Primary Care Physicians, have primary reporting relationship with their clinic Chief. Additionally they have relationships with their Regional Assistant Medical Director (RMD) Primary Care, and Department Head.

 

Key Working Relationships:

To accomplish the intended outcomes, Primary Care Physicians must maintain effective working relationships with:

Primary Physicians in their clinic

Support Staff in their clinic

Site leadership in their clinic: Their Chief, Business Systems Supervisor, Care Delivery Supervisor and other support staff

Regional Assistant Medical Directors of Primary Care (RMD) and Regional Clinic Managers (RCM)

Associate Medical Director, Primary Care

Department Heads

Consulting Physicians

Patients

 

Required Qualifications

M.D./D.O. degree and current Minnesota license

Board certification in primary care or related specialty within 2 years of joining

Knowledge of principles of managed care and evidence based medicine and the application in clinical practice

Effective communication skills and ability to work as part of a Prepared Practice Team (PPT)

Understanding and support of quality improvement principles

 

 

 

 

Position Duties & Responsibilities:

To pursue perfect care, service and access at an affordable cost, following the STEEEP principles of safe, timely, effective, efficient, equitable, patient centered care.

 

 

Key Tasks:

 

Clinical practice

review patient’s medical records

conduct medical exams and perform procedures

develop and communicate treatment plans

communicate with patient, their families and significant others

review test results

document and code all services provided

proficiency in appropriate technologies including EMR

access and productivity compliance within expectations of primary care division

active application of evidence based practice management

potential role for teaching

 

 

Involvement in Clinic Initiatives

lead and actively support clinic-based initiatives to improve medical management (e.g., pharmacy, referrals or hospitalization, etc.) and practice management (e.g., advanced access, financial performance at clinic, etc.)

active lead of a prepared practice team

participate in care unit and clinic functions (attend meetings, prepare materials for presentation, engage in discussions)

participate in department functions

-attend meetings

participate in HP functions

-attend “all MD” meetings

 

 

Professional development

maintain fund up-to-date knowledge in specialty

develop professional goals

participate in performance reviews for self review, care delivery team, site leadership and RMD
