JOB DESCRIPTION
Community Health Center of the North Country is a Federally Qualified Health Center (FQHC) located in Upstate New York. CHCNC provides high quality healthcare services to the surrounding communities and offers a sliding fee scale. With four locations, CHCNC is a growing healthcare organization that offers primary, acute, and pediatric medical care, dental services, physical therapy, and behavioral health services to the local communities. We are seeking experienced and compassionate Advanced Practice Providers to join our team at two of our health center locations. The Physician Assistant or Nurse Practitioner will work as part of the Health Center team performing a variety of direct patient care functions and formulating care plans. The Physician Assistant or Nurse Practitioner shall provide high-quality comprehensive medical care to patients across a wide range of ages, diagnosing illnesses, managing chronic conditions, and referring patients to specialists when necessary, while building long-term patient relationships and addressing overall health needs. The Physician Assistant or Nurse Practitioner will work collaboratively with other healthcare professionals to ensure the highest quality of care for our patients.

Primary Duties and Responsibilities
+ Assess and institute appropriate care for patient within established guidelines reflecting the standard practices of primary care.
+ Conduct initial examinations with recommendations for treatment and follow-up care to individuals.
+ Order routine and diagnostic lab and x-ray studies.
+ Diagnose and manage acute and chronic illness.
+ Formulate, initiate, and monitor patient care plans in conjunction with the supervising physician, when necessary.
+ Review and maintain all patient records on a regular basis to comply with required standards.
+ Prescribe medications within the appropriate scope of licensure.
+ Ensure referral and/or consultation with a collaborating physician, as appropriate.
+ Maintain proficiency in using eCW, the EMR platform, for the documentation of patient care.
+ Maintain confidential medical records.
+ Work cooperatively and supportively with all other staff.
+ Serve as a member of the interdisciplinary health center team to ensure that the medical needs of patients are met.
+ Assure compliance with all regulations of the Health Center and its services.
+ Perform other necessary duties as required by CHCNC to meet the goals of providing primary health care services.
Requirements
+ Current, unrestricted New York State PA or NP license
+ Current, unrestricted DEA license
+ Current Board certification
+ Current BLS Certification
+ Ability to work collaboratively with other healthcare professionals
+ A strong commitment to providing compassionate and patient-centered care
+ Minimum two years of experience as PA or NP required
