This model promotes a culture of patient centeredness and staff resilience. + The team is made up of 5.15 FTE Physicians, 6.20 FTE APC’s, RN, CA, IBH, Pharm D, PSA and 20.47 FTE additional staff. + M-F; patient contact hours is 36 + Hours 7am-5pm M-F + Estimated patients per day: 20 + EPIC medical record + Flexible starting FTE (0.6-1.0 FTE) + Call = After hours, phone only (nights & weekends).
