* POSITION STANDARDS * Unrestricted NH Medical License Required * Current DEA license Required * BLS Certification Required * Board Certified or Board Eligible in Family Medicine. If Board Eligible, must be actively working toward Board Certification. * Demonstrated Competence in providing comprehensive family medicine as delineated on clinical privilege list * POSITION PHYSICAL REQUIREMENTS * Anything listed here requires a pre-employment physical by Employee Health to determine if the employee is capable of meeting the requirements. * Physical Activity: * Upper Extremity: * Push/Pull/Lift/Carry: * PART TWO:FUNCTIONAL RESPONSIBILITY * Position Objective * Provides continuing and comprehensive primary care to adult and pediatric individuals and their families. * Performance Expectation * Evaluate all assigned patients presenting to the Primary Care Clinic. * Obtain an appropriate history and physical exam. * Order and interpret diagnostic laboratory and imaging studies. * Provide appropriate treatment to all assigned patients presenting to the Primary Care Clinic. * Obtain consultation as clinically indicated and appropriate. * Document the history, physical exam, and laboratory and imaging studies as well as treatment in the medical record. * Obtain appropriate informed consent for all procedures as described in APD Medical Staff Bylaws, Rules and Regulations and Policies. * Complete all outpatient hospital records per APD bylaws * Provide consulting providers with complete and accurate medical records. Communicate in a timely and collegial manner. * Review previously ordered laboratory and imaging studies and determine appropriate course of treatment for patient based on results. Communicate results to patients in a timely manner. * According to specialty, Provide call coverage for call group's established adult and pediatric patients. * Provide "back-up" call coverage for hospitalist service between hours of 7PM and 7AM and on weekends. * Document all "after-hours" communication with patients * Treat all patients and staff with respect. * Participate in staff education and clinical Quality Assurance/Performance Improvement activities. * Attend clinic staff meetings when requested to do so by the Primary Care Medical Director or Practice Manager. * Abide by CMS Conditions of Participation for Hospitals. * Abide by the APD Medical Staff bylaws, Rules and Regulations and policies. * Abide by all APD policies * Area of Interest:Physicians/Providers; * Work Status:M-F; * Employment Type:Full Time; * Job ID:5376 Dartmouth Health is an Affirmative Action and Equal Opportunity Employer.
